Who we are
Azabudai United Dental Office® (Azabudai,Kamiyacho, Roppongi) is the English speaking dental clinic with state of the art facilities to provide both routine and emergency dental treatment in an efficient and comfortable way. We incorporate CAMBRA system (CAries Management By Risk Assessment) in our prevention program. Our doctors are both graduate of US dental schools and licensed in the United States. The clinic meets the requirement for International Expatriate Insurance company and is direct provider for Cigna and Allianz. Our services include Children Dentistry, Implant, Cosmetic Dentistry, including Dental Emergency, Routine Check-up and Cleaning, Fillings, Root Canal Treatment, limited Orthodontics (Invisalign) and CAMBRA. We provide all printed documentations in English with ADA procedures codes acceptable for international insurances.
New Patients Information
Our fee for Limited Exam for emergency patients with one small x-ray and Oral Photo is JPY22,000 and we may need additional test/x-rays to give you accurate diagnosis and do any treatment.MYAppointment request and MyChart Account
The first step towards a beautiful, healthy smile is to schedule an appointment.
MyChart, our online portal allows you to fill out your patient information and complete any future updates without having to worry about sending emails or forgetting your forms at home.
We will need to have an e-mail address where we can remind of the appointment a mobile phone number where we can get in touch with you, your birthdate to register as our patient. If you have any specific issues, please send the photo of the tooth. If you have Cigna, Allianz, or Globality DKV, please send us the photo of both side of your insurance card by mail, so we can verify your eligibility before you arrive. We will hold your appointment for 15 minutes. You will receive appointment reminder through mail. Please press the Confirm bottom or let us know to change your appointment. Payment Options
For your convenience, we accept cash, and most major credit cards. Payment is expected at the time services are performed.
Dental Insurance
We do NOT accept Japanese National Health Insurance. We provide fee-for-service treatment. Please refer to FAQ for the cost of examinations and treatment.
We accept direct payment for the following international health insurance companies.
Cigna International
Allianz
Globally DKV.
We can claim directly, but the patient is responsible for any balance. If you want us to do direct claim for you, you must agrees to leave Credit Card Information to allow us to deduct any balance not paid by the insurance company. Patient not residing in Japan or leaving soon will need to pay in full before the start of treatment and claim themselves.
For all other type of insurances, we will try to help patient to fill the forms. All our forms and receipt are in English and coded with ADA codes.
